PARENT /GUARDIAN AGREEMENT The parent/guardian of the above-mentioned player hereby consent to his/her playing in the Hockey Regina Inc. (HRI) minor hockey system. I/we do hereby release HRI and its officers from all liability in respect to any injury sustained by my son/daughter while playing hockey on any sheet of ice under the jurisdiction of HRI. I/we further agree to accept financial responsibility for any equipment supplied to my son/daughter by HRI that is lost or damaged while in his/her custody and to return such equipment clean and in good repair to Hockey Regina Inc. The parents/guardians and players further agree to be bound by the terms and conditions of HRI Bylaws, Rules and Regulations and to adhere thereto.

UPON PLACEMENT WITH A HOCKEY REGINA INC. TEAM, YOU WILL BE REQUIRED TO PROVIDE THE FOLLOWING:

A copy of the player’s birth certificate if you were not registered with Hockey Regina Inc last season

A copy of the parent’s or guardian’s drivers license if requested

Registration fee. Players will not be registered with the Saskatchewan Hockey Association (SHA) until registration fee is paid in full.

In an effort to create equal teams the league may not be able to accommodate requests. Refund requests must be made in writing and will be prorated based on the number of games played less SHA registration fees and $10.00 for evaluation and administration expenses. No refunds will be made after January 10, 2000. Fees for player additions to rosters must be paid in full before registration with the SHA will be made.
